Supportive Housing Eligibility Criteria:
- A mental health diagnosis; may have cooccuring disorder
- People who express an interest in learning and developing skills to promote community integration and to live independently in their community
Services Include:
- Housing Assistance
- Community Integration based on Natural Supports
- Restoration of Daily Living Skills
- Social skills to develop personal and natural support networks
- Negotiating landlord/neighbor relationships, tenancy law as well as tenancy obligations
- Wellness and healthy lifestyles
- Securing and renewing entitlements
- Career Development (Employment, Volunteer, and Education Readiness)
- Medication Education / IMR / Advanced Directives
